Duties Description Coordinate the development of individual service plans and the delivery of services to consumers with physical and/or cognitive disabilities with the primary goal to assist the consumer in attaining/retaining optimum employment or daily living and/or social skills.
Coordinate the resources of family, community, service providers, educational institutions, and employers to further the attainment of the NYS Commission for the Blind consumer’s program goals.

Minimum Qualifications Transfer: One year of permanent competitive or 55b/c service in this title or in a title eligible for 70.1 transfer.
Open to the Public: This title is part of the New York Hiring for Emergency Limited Placement Statewide program (NY HELPS) *. For the duration of the NY HELPS Program, this title may be filled via a non-competitive appointment, if the candidate meets the below NY HELPS minimum qualifications:
A current Commission on Rehabilitation Counselor Certification (CRCC) certificate; OR a master's degree in rehabilitation counseling, including a supervised internship, from a CORE (Council on Rehabilitation Education) accredited program; OR a master's degree in rehabilitation counseling or counseling and notice of eligibility to sit for the Commission on Rehabilitation Counselor Certification (CRCC) certificate examination; OR a master’s degree or higher in Rehabilitation Counseling; OR a master’s degree or higher in a related degree field AND two years of professional work experience in a related field.
