Unser Ziel ist eine möglichst selbstständige Lebensführung. In unserem multiprofessionellen Team begleiten wir die Rehabilitand:innen auf ihrem Weg zu mehr psychischen Stabilität, Lebensqualität und gesellschaftlicher Teilhabe. Ihre Aufgabenschwerpunkte: Sie betreuen unsere vorwiegend Rehabilitand:innen im Rahmen der Einzelfallhilfe als feste Bezugsperson. Sie konzipieren und führen sozialpädagogische Gruppenangebote durch. Ihre sozialpädagogische Arbeit dokumentieren und evaluieren Sie sorgfältig. Sie bringen sich aktiv in die interdisziplinäre Zusammenarbeit im Team ein.
