Position Overview We are seeking a highly motivated and experienced Referral Pathway Specialist to join our team. The ideal candidate will have a proven track record of building and maintaining strong relationships with General Practitioners (GPs) and clinical cardiologists. This role is crucial in helping interventional cardiologists create a robust patient pool through effective referral networks and patient identification strategies. Essential Job Functions Partner with interventional cardiologists to educate them on their patientu2019s needs and assist/guide them in utilizing their patient databases with specific criteria to identify eligible patients/potential candidates from their own practice. Provide support to interventional cardiologists in identifying and addressing any challenges related to patient referrals. Develop and implement strategies to create and maintain a comprehensive patient referral network. Establish and nurture relationships with General Practitioners, clinical cardiologists, and other relevant healthcare professionals. Coordinate and facilitate communication between GPs, clinical cardiologists, and interventional cardiologists to ensure seamless patient referrals. Organize and conduct outreach programs, meetings, and presentations to promote referral network initiatives. Monitor and evaluate the effectiveness of referral networks and make necessary adjustments to improve outcomes. Maintain accurate records of referral activities and generate regular reports on referral patterns and outcomes.
